The lecture "A Culture towards Safety" presented the Hospital Mãe de Deus' experience on the theme. Based on evidences of low-quality studies, terminology problems, few negative studies and multiple-action programs (bundle), a work to enable a highly reliable organization was coordinated. Therefore, the culture towards safety aims at defining the leadership structure and systems; identifying and minimizing risks and dangers; evaluate the culture and encourage teamwork. The goal was to identify and mitigate risks and dangers; to analyze generic risks and the efforts towards specific risks; to structure a follow-up system that unveils the safety problems and to create indicators. The challenges we face today show that the current methods are still highly dependent on surveillance and hard work and that the focus on the outcome tends to create an over exaggerated feeling of reliance, causing an unreal sensation of safety.
