Summary: Patient Safety - How Do I Do It? Marco Aurélio Vitorino Cunha

The lecture "Patient Safety - How Do I Do It?" presented Hospital Estadual de Diadema's experience regarding the establishment of a culture towards safety. The lecturer presented the strategies adopted by the institution to evaluate the different perceptions concerning safety in the various hierarchic levels of the hospital. Such evaluation is undertaken through an AHRQ tool and aims at leading actions to solve weak points and understand the strong ones. As a result, they saw a 16% improvement in non-punishable response to the mistake and a 13.5% in relation to internal transferences and shift changes.
