The lecture "Managing the Care in the Institutions and the Net of Services" discussed the issue of sustainability in the health system having as background the demographic and epidemiologic transition of the Brazilian population over the next fifteen years. Therefore, it was proposed an approach regarding the course of life with emphasis on active aging, maintaining independence and preventing incapacities. The lecturer also pointed out the main problems concerning our health system, among them: the focus is on healing the disease and not on taking care of the health, fragmentation of healthcare, the system-financing model, among others. Therefore, considering an integrated model showing the experience of VIVA, integrated model. After two years of implementation, there was a reduction of 38% in admission costs, the average of permanence in the hospital was 50%, lower than the group that didn't suffer any intervention, the number of doctor appointments is significantly high, confirming the strategy of establishing a relationship with the assistant doctor, and it was observed a difference of 11,2% between IPCA e VCMH.
